Management of Personal Health Information
The practice has strict guidelines on confidentiality, patient consent, consent for a 3rd party to obtain information, transferring of health information and access and security of personal health information.  It adheres to The National Privacy Principles Act and the Health Records and Information Act.

URGENT RESULTS
Your Doctor or the Practice Nurse will attempt to contact you immediately via contact details supplied. If our attempts at contact are unsuccessful, a letter will be forwarded to your address to contact us urgently for an appointment.

NON URGENT RESULTS
The Nurse will contact you via phone to advise that you need to make an appointment to see your Doctor. If our attempts to contact you are unsuccessful, a letter will be forwarded to your address to contact us for an appointment.

If your results are NORMAL we will NOT contact you. You are welcome to phone us to check if your results have been returned to us, but you will have to make an appointment to discuss these results.
At no time are results provided to you over the phone due to confidentiality laws.

Patients are contacted for recalls and reminders. If our attempts at contact are unsuccessful a letter will be forwarded to your address to contact us for an appointment.

(Confidential information is not included in these letters). All personal information is stored on a program specific, encrypted computer data base

CONTINUOUS EDUCATION
The practice is committed to supporting the doctors, nurses and administration staff to participate in on-going education.


APPOINTMENTS
Please ring 08 9576 1091 for an appointment.
If you need more time with the doctor, please tell the receptionist when you make your booking. Urgent medical problems will always be dealt with promptly.

Fees
* Chittering Health Service is dedicated to providing you with excellent health care as we are a bulk billing practice for all patients who hold a valid Medicare card (with some exceptions).
* Patients without a medicare card are required to pay a fee on the day and all workers compensation/motor vehicle claims are to be paid by the patient, until such time as the claim has been accepted by the insurance company.
* Standard Consult (15 min) - $70.00
* Long Consult - (20min - 30min) $110.00
* Commercial Drivers Licence - $132.00
* Workers Compensation - approx $135.00 upwards, depending on consult time.
* A small fee may also be applicable for sundries, used during minor procedures.


CANCELLATIONS / MISSED APPOINTMENTS
We require 2 hour’s notice if you wish to cancel your appointment and if you miss your appointment a non- attendance fee of $20.00 will be charged.
Surgical procedures and wound dressings attract an extra fee. (ask reception before procedure)


TELEPHONE AND ELECTRONIC COMMUNICATION
Electronic Communication is available via email at reception@chitteringhealth.com.au.  All personal email is stored on a program specific encrypted computer data base system. Any emails or phone calls addressed to clinical staff will be forwarded to the appropriate physician and returned with a phone call.

Dr Eirini Habib (MBChB)


Dr Eirini graduated from Medicine from Alexandria University, Egypt in 1992 ,Has been working since then as a GP across Egypt , the Emirates and Australia She has vast knowledge across a broad range of general medical conditions, with a particular interest in Women's health, children health and nutrition .

Dr Eirini is also fluent in Arabic and available for consultations on Monday, Wednesday and Friday




Dr. Naguib Youssef MBBch

Dr Youssef is a General Practitioner from Egypt. He was graduated in 2000.
He has a special interest in family health care. He enjoys spending time with his family and is passionate about music and painting

Areas of interest include:

  • Healthy Kids and Elderly Health Assessments
  • Chronic Disease Management e.g. Diabetes, Asthma, Hypertension, etc.
    Via Care Plans and EPC allied health referrals

Dr Peter Micheal Cudjoeon MBBch

Dr Michael Cudjoe is a General Practitioner with over 10 years of medical experience in Ghana and Australia.
He has a special interest in Mental Health, Chronic Disease Management and Indigenous Health.

Karen Alford (Receptionist)

Karen has been living in Upper Chittering and loves the country lifestyle. She used to visit her Uncle’s farm in Bindoon many years ago and never did she think she would become part of the community again, some 37 years later.  Whilst this is the beginning of her journey working at a medical practice, Karen has several year’s experience in customer service, as well as working for a leading health insurance fund in W.A.




Naomi Carr (Practice Nurse)

Naomi is a comprehensive registered nurse.  She graduated from Edith Cowan University Churchlands, Perth in 2002.  She spent 10 years working in adolescent mental health, before making the move over to practice nursing.  She lives on a farm with her husband, running cattle and enjoys horse riding.


Mary Angus (Practice Nurse)

Mary is one of the Practice Nurses at the new Chittering Health service in Bindoon. She has been a nurse for 39 years with her early years being spent working in aged care and since the late 80’s working in the Operating Theatre & Recovery. She moved to the shire of Chittering about 7 years ago and loves working with the local community in General Practice.
